Orthodontic preparation with elimination of TMJ dysfunction and bite normalisation

The before/after results are impressive. This transformation was achieved by normalizing the position of the lower jaw and bite height.

The patient initially consulted an orthodontist with complaints of a misaligned bite and facial asymmetry. She also reported a mild clicking sound on the right side when closing her mouth.

An MRI scan was performed, revealing a slight disc displacement on the right side, while the disc on the left side was in its correct position.

A comprehensive diagnostic assessment was conducted, including a minor functional analysis—a complex set of diagnostic procedures necessary for gathering complete data to develop an accurate treatment plan.

This analysis included:
1. Muscle palpation to assess muscle tone and detect functional abnormalities.
2. Cephalometric X-ray analysis to obtain precise measurements of the upper and lower jaw bones, calculate tooth inclinations, and evaluate facial height.
3. A full photo protocol of the teeth and face for further analysis.
4. Impression taking or intraoral scanning to create models for laboratory analysis, followed by mounting them in an articulator to assess occlusion not only in the closed position but also during various jaw movements, evaluating both external and internal aspects.

After this, condylography was performed.

Condylography is an instrumental diagnostic method for assessing the temporomandibular joint (TMJ). It provides a graphical recording of lower jaw movements, specifically tracking the joint path relative to the hinge axis (the center of the joint head).

This step is part of a more extensive functional analysis performed for patients with TMJ dysfunction. The most common complaints among these patients include clicking in the joint, pain when opening the mouth, chronic headaches, muscle spasms, and jaw locking upon opening, often caused by a misaligned bite.

Based on the diagnostic findings, a repositioning splint was fabricated, followed by overlays on the teeth to maintain the correct jaw position.

Only after stabilizing the jaw position was the orthodontic treatment initiated with the placement of braces.
