Orthodontic treatment for impacted and palatally erupted tooth

The patient presented with a concern about an unattractive smile: one tooth had erupted on the palatal side in a second row, and there was a complete lack of space in the dental arch for its proper positioning.

The patient was concerned about the appearance of the smile as well as discomfort during chewing. One canine had erupted on the palatal surface — “in a second row,” indicating impaction and a pronounced space deficiency. The facial and dental midlines were not aligned, and the bite was displaced, which over time could have led to increased tooth wear and additional stress on the temporomandibular joint.

After diagnostic evaluation and imaging analysis, it became clear that without creating space in the arch, the tooth would not be able to move into the correct position on its own. An individualized orthodontic treatment plan was developed, and a braces system was placed.

To reposition the impacted tooth, a mini-implant was used. This allowed space to be created in the dental arch in a controlled and safe manner. Subsequently, the arch form was gradually normalized, the teeth were aligned, and the bite was corrected.

The tooth was successfully brought into the dental arch, smile aesthetics were restored, the dental midlines became symmetrical, and the bite achieved functional alignment. The patient noted a significant increase in confidence when smiling.
