Clinical Situation
Before placing braces or clear aligners, it is critically important to perform full oral sanitation in order to eliminate any hidden sources of infection and areas of weakened enamel.
During the clinical examination, a pigmented fissure was detected on the occlusal and buccal (vestibular) surfaces of tooth 3.7. During diagnostic probing, the explorer became caught in the fissure and caused painful sensations — a classic sign of an early carious lesion.
Examination and Treatment Planning
The dentist performed a thorough examination using a dental microscope.
This allowed precise identification of the boundaries of the carious process and enabled planning of a максимально conservative intervention without removal of healthy tooth structure.
Treatment Stages
- Under local anesthesia, the carious tissues were carefully removed while preserving healthy enamel and dentin.
- The cavity was prepared and treated with consideration of the fissure anatomy and morphological features.
- For restoration, a high-quality composite material was used, shade-matched to the natural tooth color and anatomically modeled according to the tooth’s morphology and occlusion.
- Final polishing and occlusal adjustment were performed.
Outcome for the Patient
Tooth 3.7 was fully restored, and discomfort during probing was eliminated.
Thanks to the minimally invasive approach, maximum preservation of healthy tooth tissues was achieved, which is especially important prior to orthodontic treatment.
The patient can proceed with brace placement without any limitations.

The presence of caries prior to orthodontic treatment may lead to progression of the lesion beneath braces or aligners, complicating and prolonging the overall treatment process.
