The patient reported sharp pain when eating sweets or cold foods, as well as discomfort while chewing on the side of tooth 1.6.
Examination and diagnosis
During the examination, the following findings were made:
- a large old filling on the occlusal-distal surface was partially destroyed;
- a chipped filling with poor marginal adaptation;
- secondary caries under the filling;
- a new carious lesion detected on the mesial contact surface.
Although the patient was in the middle of orthodontic treatment with aligners, additional therapeutic work on the tooth did not require any changes to the orthodontic plan.
Treatment plan
- Remove the old, unsuitable filling.
- Completely eliminate carious-affected tissues.
- Prepare the cavity with minimal intervention in healthy structures.
- Restore the tooth using a modern composite material closely replicating the natural anatomy.
- Perform occlusal adjustment and final polishing.
Treatment steps
- Topical and infiltration anesthesia were administered to ensure complete comfort.
- The old filling was removed, and all carious tissues were cleaned.
- The cavity was prepared with properly defined boundaries.
- A layered composite restoration was performed, accurately reproducing the anatomy of the chewing cusps of tooth 1.6.
- Occlusal adjustment was carried out, taking into account tooth positioning within the aligners.
- Finishing and polishing of the tooth surface were completed.
Result
- Sensitivity to sweet and cold stimuli was completely eliminated.
- The anatomical shape and chewing function of the tooth were restored.
- Further tissue destruction and risk of complications were prevented.
- Orthodontic treatment with aligners continued without any interruption, as the restoration integrated seamlessly into the overall plan.
Why this is important
Old fillings, especially large ones, often become sources of recurrent caries and pain. If such a filling is not replaced in time, the decay can progress deeper, leading to more complex and expensive treatment. In such cases, it is important not to delay a dental visit, even if you are already undergoing other treatment, such as orthodontics.
At DentalDate, patients can receive orthodontic treatment with aligners and restorative therapy simultaneously, without disrupting the overall treatment plan. This comprehensive approach saves time, prevents complications, and ensures not only a beautiful smile but also the long-term health of the entire dentoalveolar system.
