Restoration of tooth 1.6 with a lesion in the context of chronic sinusitis

The patient came to the clinic on referral from an ENT doctor because she had chronic right-sided sinusitis.
Suspicion of odontogenic cause of the disease.

Objectively: 1.6 old filling, broken marginal adherence, caries on the medial wall. Percussion is weakly positive on the entire right side. At the moment sinusitis is exacerbated, with accompanying symptoms. She is under therapy for this condition.

On CBCT: The tooth was previously treated endodontically in another clinic. The medial cheek root has a complex curve in the apical third. The canals are obturated to the apex. In the periapical tissues of the medial cheek root there is a focus of radiolucency, the contours are not clear.

It was decided to re-treat the canals of tooth 1.6.

The old filling was removed. Access to the root canals was created. The root canals were unsealed. Calcium hydroxide was placed in the canals for 2 weeks for the purpose of disinfection inside the root structure.

After 2 weeks the canals were mechanically and medically prepared for obturation and filled by the method of hot condensation of gutta-percha and siler AH plus. Filling.
Treatment was performed under dental microscope.
